Athletic FHSAA Forms
Tampa Catholic Athletic Forms are going all digital
We will be using www.AthleticClearance.com for all of our athletic forms. All directions, including a tutorial video (please watch before starting the process) are available on the Athletic Clearance website to help you navigate through this new process.
PARENTS: Please visit www.AthleticClearance.com and choose your state. Watch a quick tutorial video, then register an account with valid email username and password. You must complete this online registration each school year before participating in any sports. This includes any athletic tryouts.
You will still need to have a physician sign the physical form verifying your son or daughter to be able to participate in athletics, but then that form will be uploaded to the site. Everything else will be done on the website. Make sure that the FHSAA EL 2 physical form is the one that you upload.
We think this will make the process much easier for all as we move away from paper forms and into the digital age. The athletics department will not accept any paper forms.
