Health Physicals

 

Note: The information presented here pertains to the 2008-2009 school year.

 

All Students New to Tampa Catholic (class of 2012 and transfer students)

Florida State Law requires that each student submit a current health/physical form and proof of immunization before they begin school.  These immunizations include the Hepatitis B-series, a second MMR, and a tetanus inoculation. It is imperative that the physical form is current. Health forms dated before March 3, 2008 will have expired.

The yellow physical form and the blue immunization form are procured at your doctor’s office.

The current health form and proof of immunization should be mailed to the school (4630 N. Rome Ave., Tampa, FL, 33603) by July 16th in order to allow sufficient time for processing. It will not be a problem if the physical arrives a day or two late but by state law no student may start school without first having submitted this important documentation.

 

Athletes – Both New and Returning Students

If you are planning on participating in any sport at Tampa Catholic, you must also submit the FHSAA Physical Evaluation and Consent forms. These forms must be dated after June 1, 2008!  It is also necessary to send a copy of your birth certificate.  Give these forms directly to your fall coach or the athletic director. No student will be unable to participate in a sport at TC until the forms are submitted.

Please note: The State health form cannot be substituted for the Florida High School Athletic Form; they are not interchangeable.
